APPLICATION
FOR A CROSSBOW PERMIT (NO FEE REQUIRED):
The Arizona
Game & Fish Dept. may issue a Crossbow Permit to those of you who have
a permanent 90% minimum impairment of one arm. This impairment must
be attested to by a physician on a form provided by the Dept. If you
meet the above requirements, you are eligible to apply for a permit.

FOR A CHAMP PERMIT (NO FEE REQUIRED):
The Arizona
Game & Fish Dept. may issue a CHAMP permit to those who have a permanent
disability or combination of disabilities, creating a minimum of impairment
of function equivalent to at least 90% in one leg, or visual field impairment
of at least 80% in the better eye, or corrected vision in the better
eye of 20/200 or less and attested to by a physician on a form provided
by the Dept. If you meet the above requirements, you are eligible to
apply for a permit.
